Top Pharmacist Job: Long-Term Care Pharmacist in Dallas

If you are a pharmacist with experience in long-term care, Rx relief has an exciting opportunity for you in the Dallas-Fort Worth area.

The person hired for this job will be responsible for evaluating physician medication orders for appropriateness of drugs, dosage, potential interactions, route of administration, and stability. They will also make recommendations for therapy changes to the physician and facility staff as needed. Other duties of the job include working with patient medical information to conduct drug utilization reviews on all prescriptions and reviewing clinical edits to avoid any adverse drug interaction.

This position also requires the maintenance of a patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication, including over-the-counter and home remedies. The pharmacist will also work with pharmacy technicians in reconciling the controlled substances log with inventory on hand and prescriptions that have been dispensed to comply with state and federal regulations.

The pharmacist will also work to ensure that implementation of cost containment strategies occurs on all dispensing and procurement activities.

The person who fills this position will also provide clinical expertise by responding to drug-related questions and concerns of technicians, nursing staff, directors, administrators, and customers. They will also collaborate with the healthcare team to provide clinical and technical support as needed.

Other duties include documentation in the medical records of all communication with physicians, nurses and other members of the healthcare team, as well as patients, residents and their families, obtaining and maintaining all required documentation in accordance with state and federal regulatory agencies, implementation of policies and procedures for the safe procurement, storage, distribution and use of drugs and biologicals, as well as disposal of drugs. The pharmacist will also assist staff with procedures relating to safe and effective methods of procuring and handling and disposing of drugs.

Requirements


This is a temp-to-hire position. The person hired must have a valid, open and active Texas license, an RPh or PharmD degree, and one or more years of recent pharmacy experience in a long-term care setting.
